So I'm coming to Charleston

to get married, basically,

and look for a hospitalist job.

And at the time, in one of Amanda's

multiple iterations in her career,

she was hospitalist, like, manager,
00:02:02:11 - 00:02:05:12
director of, like, the nursing stuff.

And so I come for the interview

and I met her and I was like, oh, my God,

she's so cool.

And I'm like, yay,

you started as a BNU nurse.

had a psychiatric diagnosis,

but were also fairly

medically sick as well.

And then you got snatched up

by the hospitalists?

That's right.

They that was a new concept at the time.

Y’all this is a long time ago.

Been a nurse for 20 years.

But the health care system
00:03:51:17 - 00:03:53:01
that we work for
00:03:53:01 - 00:03:55:24
did not have a hospitalist program.
00:03:55:24 - 00:03:57:02
And so at one point
00:03:57:02 - 00:03:59:01
they contracted someone
00:03:59:01 - 00:04:00:07
and they were great
00:04:00:07 - 00:04:01:22
and they decided, hey,
00:04:01:22 - 00:04:02:12
wait a second,
00:04:02:12 - 00:04:04:04
we should just own the practice.
00:04:04:04 - 00:04:06:04
So when they started that practice,

they asked me to sort of move

into a coordination

role for the clinical piece,

which I absolutely loved.

To this day,

I still say it was my favorite job

Um no, and I always

now I call myself

a reluctant administrator

because I never set out in health
00:05:06:08 - 00:05:09:11
care to be in any kind of leadership role
00:05:09:11 - 00:05:12:01
or any kind of administration position.
00:05:12:01 - 00:05:13:21
And I thought that I would just
00:05:13:21 - 00:05:15:11
go to the hospital and work
00:05:15:11 - 00:05:16:17
three shifts a week,
00:05:16:17 - 00:05:18:10
and that was it,
00:05:18:10 - 00:05:20:23
you know, And then one day I'd retire.
00:05:20:23 - 00:05:23:01
But unfortunately,
00:05:23:01 - 00:05:24:13
that's not what happened.
00:05:24:13 - 00:05:26:12
I mean, fortunately or unfortunately,
00:05:26:12 - 00:05:27:06
that's not what happened.

But I didn't realize

I've always been organized.

I went to college

on a leadership scholarship.

You know, I did not know that, that I did

I did nerd alert and I.

That's not that's not a secret.

I had no idea that I was so good

at programmatic development, I guess.

Right from until I got into it.

You know, I'm a problem solver.

And that's really what

programmatic development

is, is just solving problems.

Plus, I grew up immersed in health care.

I'm the third generation

person

in my family to do public health.

So I lived in that world.

I grew up in that world of how
00:06:12:05 - 00:06:13:17
to solve these kinda problems.

And, you know, my dad
00:06:14:24 - 00:06:16:09
practiced rural medicine
00:06:16:09 - 00:06:18:02
and I watched him my whole life.
00:06:18:02 - 00:06:20:22
So I like that problem solving

aspect of the of the job, for sure.

Now now the the next piece of the puzzle.

When she left me before I really arrived.

But I mean,

this is really

a cool part of your career story.

So hospitalist director and I think

that role really grew.
00:06:43:03 - 00:06:44:16
Over how many years did you do that
00:06:44:16 - 00:06:45:03
for the hospital?

I think I was there
00:06:45:17 - 00:06:46:22
for about seven years.
00:06:46:22 - 00:06:48:11
I did that role
00:06:48:11 - 00:06:50:17
and sort of did a hybrid of that role.
00:06:50:17 - 00:06:53:04
And then my next role
00:06:53:06 - 00:06:54:18
when that came to fruition.
00:06:54:18 - 00:06:55:23
But yeah, for seven years
00:06:55:23 - 00:06:57:12
I was with the hospitalist.
00:06:57:12 - 00:06:59:05
Okay, so then tell everybody
00:06:59:05 - 00:07:01:07
what the next thing was.
00:07:01:07 - 00:07:03:14
Another big iteration.
00:07:03:14 - 00:07:06:01
Yeah, a huge, huge iteration.
00:07:06:01 - 00:07:07:07
You know, part of
00:07:07:07 - 00:07:09:04
if anyone out there in listening

land is in the hospitalist world,
00:07:10:23 - 00:07:12:24
they know that length of stay
00:07:12:24 - 00:07:13:19
and readmission
00:07:13:19 - 00:07:17:14
rates are metrics that we have to
00:07:17:16 - 00:07:19:04
pay attention to. Right.
00:07:19:04 - 00:07:20:04
And part of my role
00:07:20:04 - 00:07:21:13
in the clinical coordination
00:07:21:13 - 00:07:23:03
was to keep patients
00:07:23:03 - 00:07:25:19
from coming back to the hospital.
00:07:25:19 - 00:07:27:21
And what I found was that we didn't
00:07:27:21 - 00:07:29:16
have a way to do that
00:07:29:16 - 00:07:30:11
in the Lowcountry
00:07:30:11 - 00:07:31:16
because we're certainly not
00:07:31:16 - 00:07:35:16
in our health care system, because
00:07:35:18 - 00:07:38:03
there's not enough access
00:07:38:03 - 00:07:40:13
to care in this area.
00:07:40:13 - 00:07:41:16
And we are well,
00:07:41:16 - 00:07:43:06
we the the hospital system
00:07:43:06 - 00:07:44:23
that we worked for at
00:07:44:23 - 00:07:45:23
the time
00:07:45:23 - 00:07:47:11
was the only not for profit
00:07:47:11 - 00:07:48:20
one in the community.
00:07:48:20 - 00:07:51:17
And so I just started thinking like,
00:07:51:17 - 00:07:54:17
what if the hospitalist,
00:07:54:19 - 00:07:56:15
there was a large number of them?
00:07:56:15 - 00:07:58:08
What if they saw patients
00:07:58:08 - 00:08:00:24
in some sort of follow up capacity
00:08:00:24 - 00:08:02:15
after they were hospitalized?
00:08:02:15 - 00:08:04:20
Perhaps that could keep them from coming,
00:08:04:20 - 00:08:05:13
keep the patient
00:08:05:13 - 00:08:07:03
from being rehospitalized
00:08:07:03 - 00:08:09:09
or utilizing the emergency department
00:08:09:09 - 00:08:09:19
in a way
00:08:09:19 - 00:08:11:12
that it didn't need to be utilized.
00:08:11:12 - 00:08:13:06
And I thought that was like
00:08:13:06 - 00:08:15:08
the best idea I'd ever had. Right.
00:08:15:08 - 00:08:16:19
And so I went to the powers
00:08:16:19 - 00:08:18:23
that be and they were like,
00:08:19:00 - 00:08:19:13
maybe.
00:08:19:13 - 00:08:19:19
And then
00:08:19:19 - 00:08:20:19
I went to the hospitalists
00:08:20:19 - 00:08:21:16
and they said, Heck
00:08:21:16 - 00:08:23:13
no, we're hospitalists for a reason.
00:08:23:13 - 00:08:25:10
We don't want to see patients
00:08:25:10 - 00:08:26:12
in the outpatient world.
00:08:26:12 - 00:08:27:06
Why would we do that?
00:08:27:06 - 00:08:30:06
We're hospitalists and I was like, okay,
00:08:30:06 - 00:08:32:08
But the stars had aligned
00:08:32:08 - 00:08:34:14
and there were some primary care docs
00:08:34:14 - 00:08:35:10
in that
00:08:35:10 - 00:08:37:07
in the system that wanted to retire,
00:08:37:07 - 00:08:38:18
but not fully.
00:08:38:18 - 00:08:40:13
And so they kind of sat on a wing
00:08:40:13 - 00:08:43:19
and a prayer like halfway through a year.
00:08:43:21 - 00:08:46:22
Okay, we'll let these guys like
00:08:47:01 - 00:08:47:24
three days a week
00:08:47:24 - 00:08:49:01
y’all can see patients
00:08:49:01 - 00:08:49:21
that are discharged
00:08:49:21 - 00:08:51:17
from the hospitalists service.
00:08:51:17 - 00:08:53:12
We'll just see how it goes.

Like in the morning,

I would do my hospitalist job
00:08:55:19 - 00:08:56:08
and then I'd walk
00:08:56:08 - 00:08:57:15
across the street to the clinic
00:08:57:15 - 00:08:59:15
and try to get it set up.
00:08:59:15 - 00:09:02:15
And in the first like three months,
00:09:02:15 - 00:09:04:12
we had prevented
00:09:04:12 - 00:09:06:10
just so many readmissions
00:09:06:10 - 00:09:09:14
and ED utilization
00:09:09:14 - 00:09:10:21
from patients that were enrolled
00:09:10:21 - 00:09:12:23
dropped by like 90%.
00:09:12:23 - 00:09:14:06
And so, yeah.
00:09:14:06 - 00:09:16:16
And so the hospital system went
00:09:16:16 - 00:09:17:08
hang on a minute,
00:09:17:08 - 00:09:19:04
she might be on to something here
00:09:19:04 - 00:09:20:22
so that I was allowed to do it
00:09:20:22 - 00:09:22:16
for five days a week.
00:09:22:16 - 00:09:27:13
And that grew into a massive multi
00:09:27:15 - 00:09:29:21
functional primary and preventative
00:09:29:21 - 00:09:32:13
care clinic for the underserved.
00:09:32:15 - 00:09:33:05
We did not
00:09:33:05 - 00:09:34:14
charge patients a dime
00:09:34:14 - 00:09:36:06
who didn't have any insurance or money.
00:09:36:06 - 00:09:38:16
We used our charitable funds.
00:09:38:16 - 00:09:39:14
I fundraised
00:09:39:14 - 00:09:41:04
for that millions of dollars
00:09:41:04 - 00:09:43:19
to support that endeavor,
00:09:43:19 - 00:09:45:19
and we just started taking care of people
00:09:45:19 - 00:09:48:13
and it grew into a large clinic
00:09:48:13 - 00:09:49:22
that still exists today.
00:09:49:22 - 00:09:52:04
Although I don't run it anymore,
00:09:52:04 - 00:09:53:12
it's still going strong,
00:09:53:12 - 00:09:54:08
and I feel like that
00:09:54:08 - 00:09:56:06
probably creating that clinic

in that area of town
00:09:57:24 - 00:09:59:21
where there was a huge need,
00:09:59:21 - 00:10:00:15
it was probably my
00:10:00:15 - 00:10:03:11
crowning jewel of my career doing that.

And then when the pandemic hit,

I remember to ask you about that.

Y'all remember the pandy
00:10:09:22 - 00:10:12:04
the here and now.

Yeah.

And somewhere in this chaotic

mix of things, I somehow convince Lil

to come and work at the clinic for

for me.

To see those patients

she used to see in the hospital
00:10:24:04 - 00:10:25:19
to see them in the clinic
00:10:25:19 - 00:10:26:07
and help me

prevent those readmissions

and help me decrease the utilization
00:10:29:24 - 00:10:30:14
in the E.R.

And I think we had a lot of

we had a lot of fun doing it for sure.

It's like

but I got to have an office, like,

next to me at a meeting

full of 500 people,

our leadership meeting, and said, Hey,

that was a Wednesday

in case anyone recalls.

I said, Hey could on Monday, let's let's.

We need to do a drive

thru COVID

19 testing site in the parking
00:11:11:08 - 00:11:12:14
lot of your clinic.

So that was a Wednesday,

and 96 hours later I opened a drive

that tested over 100,000 people.

some people actually really enjoy

all this is going around.

and administrative backgrounds

have better clinical outcomes

because of that care.
